Terms and Conditions - Client Bookings
– I am embarking on a course of Coaching, Hypnotherapy, Psychotherapy, NLP and/or healing from Spartan Resilience.
– My coach/therapist is qualified, insured, enhanced DBS checked, registered with the appropriate professional bodies, receives regular supervision and attends Continuing Professional Development (CPD) to keep up to date with the latest neuroscience developments.
– I will always be treated with respect and care.
– There are no guarantees of change although full engagement will greatly enhance the success of the coaching/therapy. As such I agree to listen to the guided audio MP3 and to complete any homework set by the coach/therapist.
– Disclosure of all information to the therapist shall remain confidential unless there are exceptional circumstances when my therapist deems it necessary to safeguard myself and others.
– Any notes made or personal details shall be held in strict accordance with GDPR (data protection) guidelines and I give informed consent to this.
– If I have any questions about my treatment or this agreement I shall ask.
– I understand that payment can be made by Cash, Card or Bank Transfer, with details provided on request.
– I agree to receiving communication regarding appointments and/or information in relation to Spartan Resilience. I may unsubscribe at any time which I understand will also remove access to the available content on the client portal.
– Cancellations: I am contractually bound to pay the fee as agreed with the coach/therapist each week at the time of booking the session and fully understand that half payment will be owning if I cancel giving less than 72 hours notice and full payment will be owing if I cancel giving less than 48 hours notice or if in the therapists opinion I am unfit to participate in a session.
I have read this agreement and accept the treatment on these terms.
Terms and Conditions - Armed Forces, Veterans and Emergency Services
There will never be any requirement to discuss any details of your service work for which you are not comfortable with, however in the highly unlikely event that during coaching/therapy I anticipate there is a potential sensitive subject arising (i.e. trauma as a result of operational activities), I will endeavour to steer the conversation away from any specific details at the earliest opportunity. I will not record any notes in relation and guarantee absolute discretion in all cases.
The concessionary rate of 10%, applies to self funding for all Armed Forces Personnel (Serving or Veteran), Members of the Emergency Services (Police, Ambulance Service, Fire and Rescue Service (FRS), Coastguard) and NHS. Proof of eligibility is required on initial payment (MOD 90, Veterans ID Card (or similar ID/Documentation), Police Warrant Card, Emergency Service or NHS ID card, Defence Discount Card, Blue Light Card etc.).
Subject to availability, funded therapy may be available at discretion following referral from external organisations.
